Overview:
The Transportation Coordinator is responsible for ensuring that the Central Health patient population is connected to appropriate transportation resources at the enterprise level. The Transportation Coordinator will be part of a team for transportation coordination operations within the Central Health Patient Navigation Center. This position will receive inbound electronic and telephonic communications from enterprise team members and patients requesting transportation services and will work with all parties to secure appropriate methods of transportation for patients with medically related needs. This position will communicate with patients via inbound and outbound telephonic communications within the Central Health Patient Navigation Center.
Responsibilities:
- Evaluate and approve transportation requests as permitted by program SOP, guidelines, and budget.
- Monitor incoming electronic and telephonic communications for transportation requests.
- Determine and coordinate best resource available for patient ride requests, which may include available ride share platforms, taxi, enhanced local transportation services via Metro Access or Capital Metro resources, wheelchair, bariatric, and nonemergent ambulance-based transportation options.
- Communicate directly with patients regarding transportation options available, within policy and program limits
- Communicate directly with both Central Health team members, and appropriate external partner organizations to facilitate and/or coordinate patient transportation to and from applicable healthcare related service locations.
- Coordinate transportation reservations with appropriate service partner at the appropriate lead times, per specific partner policy.
- Communicate with Lead transportation coordinator and management any issues identified while working transportation requests.
- Other related transportation service coordination and deployment of resources duties as assigned
- Ability to relay educational information effectively to patients in a clear and concise manner.
- Develop and maintain appropriate relationships with internal and external stakeholders who will utilize centralized transportation coordination services.
- Develop familiarity and expertise with assisting patients and partners in navigating available transportation resources.
- High level of attention to detail and organization will be necessary to ensure patients obtain transportation to and from appropriate locations.

Qualifications:
MINIMUM EDUCATION: High School Diploma or equivalent.
PREFERRED EDUCATION: Associate’s Degree or equivalent in related field.
MINIMUM EXPERIENCE: Experience working in a multi-cultural, community-based setting for at least 2 years
PREFERRED EXPERIENCE: Experience working in a multi-cultural, community-based setting for at least 3 years, with at least 1 year focused on transportation coordination or logistics
